All students must submit a final official transcript to the Office of Graduate Studies at graduateadmission@saintmarys.edu before starting their course work. Please note, in order for a transcript to be considered an official transcript, it must be sent directly from the registrar's office to our office. The transcript cannot come from the student.
All students must submit their proof of insurance and vaccination records to the Saint Mary's Health and Counseling Center. Directions for setting up your account can be found by clicking here. Once you are registered please complete the following:
1 - Go to the "Forms" tab and complete:
- Health History (self-reported history; you do NOT need a physical exam)
- Privacy Policy (must be signed by the student AND parent if not yet 18 years old)
- Genoa Pharmacy Consumer Enrollment Form and signed Notice of Privacy Practices
2 - Go to the "Immunizations" tab and enter all dates for the required immunizations, which include:
- MMR (2 doses)
- Tetanus within the last 10 years
- Hepatitis-B (3 doses)
- Varicella (chicken pox) - either history of the disease or 2 doses of vaccine
- Meningitis (ACWY, Menomune, Menactra, Menveo - 2 doses)
3– Go to “Upload” to upload:
- Your immunization records
- CDC COVID vaccination card
- Insurance card (both sides)
