Ready to gather registrations for your event? Use this page to request an enhanced registration form from Campus and Community Events.

Form requests should be made at least ten business days before publishing. If you have questions or need additional information please contact Eylse Paul at epaul@saintmarys.edu.

Is there a limit or cap to the number of event participants? If not, leave blank.
Please provide basic text that will appear in the body of your registration form about your event. You may want to include who to contact with more questions or what the next steps are.
Tracking Registrations
Carefully type or paste in the email address(s) for those who should receive alerts when a new registration is received. If more than one, please separate by commas.
Here is where you will provide important contact information and instructions regarding event registrations. You'll need to know WHO should get copied on each registration, and WHAT DEPARTMENT should have access/download privileges for the registration list.

Who do you want the confirmation email to come from?
If the recipient of the confirmation email hits reply, whom should receive the email? This should be someone who can answer questions about the event and handle the influx that may result.
